We offer a wide range of human services for clients receiving aid from the Department of Mental Health, so they can get back on their feet. These include:
- Job placement
- COVID-19 relief
- Medicaid coverage options
- Family planning and group conferencing
We've partnered with other organizations in the area, like the Department of Social Services, SC Stay Plus, the Department of Housing, and the COVID-19 Relief Fund Distribution program, to meet our clients' needs.

From the first assessment to ongoing advocacy, our case managers walk alongside every member through five connected stages of care.
Assessment & Eligibility
We complete the official Medicaid long-term care assessment to determine:
- Medical needs
- Functional limitations
- Safety concerns
- Eligibility for waiver programs (HCB, Ventilator, HIV/AIDS, etc.)
Care Planning
We create a person-centered care plan that outlines:
- What services the member needs
- How often they need them
- Which providers will deliver them
Service Coordination
We arrange and coordinate services such as:
- Personal care
- Respite
- Adult day health
- Home modifications
- Medical supplies
- Transportation
Monitoring & Follow-Up
We conduct regular check-ins to ensure:
- Services are being delivered correctly
- The member's condition is stable
- The care plan still fits their needs
Advocacy & Problem Solving
We help members:
- Resolve issues with providers
- Report concerns
- Request additional services
- Navigate Medicaid rules
